RECURRENT PREGNANCY LOSS & IMPLANTATION FAILURE PROGRAM

Miscarriage is the most common complication of pregnancy. Approximately 10-15% of all first pregnancies miscarry. For most couples, there is a similar risk of a repeat miscarriage. It is estimated that between 2 and 5% of reproducing couples suffer from recurrent pregnancy loss, usually involving 3 or more miscarriages. Miscarriage of a pregnancy is a physically and emotionally challenging ordeal. When pregnancy loss is repetitive, these feelings are magnified. In the past, few couples could be diagnosed with a specific cause for recurrent miscarriage. Some progress has been made in recent years in understanding recurrent pregnancy loss and providing improved diagnostic tools and treatment options.

COMPREHENSIVE DIAGNOSIS

An important component in treating couples with recurrent pregnancy loss is establishing the correct diagnosis or cause. Causes of recurrent miscarriage or implantation failure include chromosomal defects, abnormalities of the uterus, hormonal deficiencies and immunologic factors. Genetics & IVF Institute medical specialists conduct a thorough clinical and laboratory evaluation of each couple to determine the cause of miscarriage. A number of tests are useful in determining the reasons for multiple miscarriages, including:

  • chromosomal analysis of both partners
  • examination of the uterus (hysterosonography, possibly other tests)
  • hormonal evaluation
  • immunologic testing
  • other studies as indicated

An immunologic cause for pregnancy loss has been suggested for some otherwise unexplained affected individuals. There is substantial debate about the best strategies for diagnosis and treatment of immunologic factors in patients experiencing recurrent loss. INNOVATIVE TREATMENTS

Treatments for recurrent pregnancy loss depend upon the cause. When an immunologic cause of recurrent miscarriage or implantation failure is diagnosed or suspected, the following treatment options are:

  • administration of low dose aspirin and heparin
  • low dose aspirin and prednisone
  • use of donor sperm, donor egg, or host uterus
  • preimplantation genetic diagnosis of the embryo to identify chromosomal abnormalities
  • combinations of these therapies as indicated

Genetics & IVF Institute staff recognize that recurrent pregnancy loss or implantation failure is a distressing problem which, until recently, has not received much attention. In the past, a cause for repetitive pregnancy loss could be identified in only a few couples. However, recent advances in the understanding of the causes of pregnancy loss have diminished the number of unexplained losses. Technologic advances have also provided the ability to identify the cause of implantation failure in other couples.

Prior to their first visit, all patients experiencing recurrent pregnancy loss or implantation failure are provided with an information packet including registration questionnaire.  
The completed questionnaire along with a copy of all pertinent medical records from personal physicians should be forwarded in advance or brought along at the time of the first visit. Specific questions related to past pregnancy loss, implantation failure, special testing and possible treatments are discussed.

Evaluation of couples with recurrent pregnancy loss or implantation failure is coordinated by Genetics & IVF Institute staff.  
Your physician will ensure that the proper studies are obtained. Upon completion of the evaluation, a follow-up consultation may be scheduled to review the test results and to discuss the treatment plan.
